Emergency transcatheter aortic valve implantation with self-expandable valve under extracorporeal membrane oxygenation for cardiogenic shock due to bicuspid aortic stenosis: A case report

Abstract Background Transcatheter aortic valve implantation (TAVI) is an established treatment for patients with severe stenosis (AS). However, the effectiveness of TAVI cardiogenic shock due to bicuspid AS, who require mechanical circulatory support, needs further investigation. Case summary A 64-year-old male patient was admitted hospital congestive heart failure secondary and left ventricular dysfunction. After admission, he developed shock, worsening his condition. The placed on veno-arterial extracorporeal membrane oxygenation support intra-aortic balloon pump, transferred our hospital. Cardiac computed tomography revealed a severely calcified type 1 valve. deemed inoperable by team. Therefore, emergency transfemoral self-expandable performed day 2. It significantly improved hemodynamic stability. finally discharged 29 without any neurological sequelae. Discussion Cardiogenic AS has poor prognosis. this case report demonstrates that could be optimal hemodynamically unstable support.
